569469 Adsense opt in box Wordpress site

Hi! I have a mini store on Wordpress in swedish language there I sell a product. I need the following fixed: * Put in an opt-in box on first page (plugin), that collects email addresses. When they put in there email they should get an automatic email with a link to download a free ebook. You get the ebook from me in pdf. The collected adresses shall be collected in admin and that I can send emails to the subscribers with a click. The subsribers should be able to unsubscribe too. My last opt-in box disappered because of the coding when I edit the page. You will see example of it in print screen. * Put in Google Adsense on all pages in the bottom, about similar subjects as I am selling. * If possible, get the form customers fill in when they choose invoice option for payment, in correct swedish letters, and the same goes for the automatic order confirmation emails.
