PHP Form Modification

I have a PHP form that is broken.? Attached is a document with a llink inside to the form. 1. I need to have the following field required ? A) NameName:? Address:? City:? State:? Zipcode:? Phone: Email: 2. The options do not appear to be working.? They, even when not selected, email the user with the word yes beside of them.? The selected items should appear in the generated email with the information beside of them.? Any options not showed, should not appear. 3. If a user hits submit, if all required fields are not filled out, a red asterick should show up and user should be prompted to fill out required items. 4.
