I'm looking to self publish a book of poems for middle schoolers, in which the subjects are younger kids having common experiences - like getting scared by a clown at a birthday party. I need a proper, written and signed intellectual property assignment agreement along with roughly 30-40 illustrations. The cover of the book would need to be designed, too.
Clarification: I'm looking for black and white images, ink is preferred but not required for consideration. The cover can be color. I don't want to see clip art. If you want to show me your existing work, that's fine, but I want it to be applicable to some degree.
The title is:

Coulrophobia
At first, it had been the usual crowd:
playmates and parents, none causing concern.
Then an interloper who was too loud
made Brett’s birthday party take a sharp turn.
He rolled on a bike with only one wheel,
honking his red nose like it was a horn.
Brett honestly didn’t know how to feel.
What did this have to do with being born?
Baffled discomfort welled up in Brett's throat
while the painted invader tossed round balls.
The alien orbs seemed to dance and float
and all the parents chimed in with catcalls.
The adults tried to keep things lighthearted,
telling the tots they should be having fun.
Brett wasn’t sure when the chaos started,
maybe all the kids just stopped being stunned.
Was it Brett who was the first to let loose,
or was it his baby sister Mary?
The spell broken, the kids had an excuse
to admit the freaky man was scary.
The playroom filled with the shrill sound of shrieks
from a collection of panicked toddlers.
Though the big red grin remained on his cheeks,
the man's eyes looked like the crowd was bonkers.
Soon, Brett’s mom spoke to the creepy weirdo,
after which he took a permanent break.
The kids' fearful tears continued to flow
until it was time for ice cream and cake.
