I'm a developer that is building and hosting a website for a client, and I need a website mockup before I begin. That's where you come in!

The client is a "doula", which is a woman who assists another woman during their childbirth, and provides support for the mother and her family after childbirth. Somewhat similar to a midwife, except a doula is strictly non-medical. The client needs a basic website to describe herself and her services, and allow potential customers to contact her.

The header will be sticky and include a logo and business name, as well as the tabs.
Ideally, the website would be split into four tabs at the top: "Welcome", "About", "Services", and "Contact".

The "Welcome" tab is the main page of the site and describes what being a doula means, mainly through images and text. I imagine that there would only be a few paragraphs there.

The "About" tab will describe the client in detail, through images and text.

The "Services" tab will describe what services the client offers, and the rates of the services, and potentially packaged deals.

The "Contact" tab will include a form for potential customers to fill out. The form will have fields for:
- First Name (text, required)
- Last Name (text, required)
- Street Address (text, required)
- Unit (apartment, suite, etc) (text, optional)
- City (text, required)
- State (option select, required)
- Zip Code (numeric, required)
- Email (text, required, validated that it's like name@domain.com)
- Phone (numeric, required, validated that it's like 123-456-7890)
- Due Date (date selector, optional)
- Preferred hospital / birthing center (text, optional)
- General message (text, optional)
- Submit button

Some other requirements:
- It must be responsive, so that it looks great and is easy to use on desktop, tablet, and phone.
- The style should be "shabby chic". See attached files for potential color schemes and images of similar sites of this style.
- Deliverables should be in a detailed format that should be relatively easy to convert to HTML & CSS.
