I'm looking to make a notepad planner.

Source files need to be delivered in 300dpi and be editable as the content is not final and may need text changes.

Font will need to be delivered with the project.
Page size A4

Page needs to consist of the following.

(Full width) Video Title:
(1/4 width) Catergory: (3/4 width) Sponsor Details:
(1/2 width) Date recorded: (1/2 width) Date Scheduled:

Then I need a checklist that the person can tick when they complete it. (1/5 width)

I need...

A 3/5 section titled "Video Concept" that text can be written. (6 lines)
A 1/5 section titled "Talking Points" that check points can be written (6 Lines)

A 3/5 section titled "Hook" that text can be written. (8 Lines)
A 1/5 section titled "TAGS" that text can be written. (5 Lines)

A full width section titled "Description" that text can be written. (8 Lines)

There may be changes later but I need to get a rough idea of the page and how it will work.
