     Dear Contest Holder, if you share Word form and if your users’ machines do not have the fonts used in the form, they will certainly lose the design of the document. Hence, PDF forms are always safe. Kindly have a look at my entry #28 . I’ve shared a demo on how my dynamic PDF form works.

  I am not looking for changes in the format. The only thing that should be changed is how the checkboxes are going to look like. Everything else should be exactly the same. Please review the attachment document for further clarification on the required fields and functions of the document.

  A new document with the form with further instruction has been upload in the project description. With regards of 3rd: each author should describe its participation in the scientific article submitted for evaluation, that's why we have those options. To make easier the process of filling the information those options instead of written should be checkbox options. At the end of the document, those check box and the authors initials should feed the Authors Participation Statement which will describe the item of the checkbox and the authors initials for those who have the specific option mark as check.

Here are 3 comments. 1st: Number of Authors[ ] {If you input number 4, then create 4 tables in the next page for 4 Authors. 2nd: First Name [text box ], Last Name [txt box ], Name Initials [txt box], Identity document [txt box], City and country [ txt box], Date [txt box/date & time], Codes of participation [ txt box]. 3rd: I do not understand.
